hmm...
original characters come to me for various reasons.
in fanfic, most of my OCs exist to fill a role either 1) not offered in the show or 2) because, for whatever reason, i don't want to use a character already on the show...
in example, in the story Resurrection is for the Unbelievers, i needed an "about the town" flunky type for Kate, who was also connected to the nonhuman underworld ... thus was born Derskingorlus, the half demon :) i'm sure i could have found someone from within the buffy or angel mythos to use, but it was just as easy to create someone :)
in my original works, my characters all start out as dreams... daydreams, musings, actual vivid night dreams... just me, closing my eyes and going, "what if"
as for their names... well, i've been a fan of fantasy/science fiction since i was a kid, and i've played dungeons and dragons since i was eight... creating names has always been something fun for me...
often, i will come up with a name and then form a character to go with it.
i suspect creating RPG characters has helped to create my original characters too... since the generation of someone you are going to embody in a game does offer the chance to expansively imagine another person :)
anyway, so that's how i come up with my original characters. :)
